Independent Living Program residents should complete an Emergency Response Contact Information Form and return it to Kristine Sackett, ILP Housing Manager, (301) 424-9560 ext. 14900.
Emergency Contact Form
An Emergency "File of Life" Form is also required. The "File of Life" is posted on your refrigerator in an easy to spot, magnetized red plastic pocket. This information is helpful to doctors or Emergency Medical Technicians should immediate Emergency transportation be required from the home. Please see Eve to pick up the "File of Life" form.
